Covid-19 Vaccine Consent Form


COVID-19 Vaccine Moderna is a vaccine for preventing coronavirus disease 2019 (COVID-19) in people aged 18 years and older. COVID-19 Vaccine Moderna contains a molecule called messenger RNA (mRNA) with instructions for producing a protein from SARS-CoV-2, the virus that causes COVID-19


The Pfizer-BioNTech vaccine is available to use for people aging 16 and over to prevent getting a severe infection by the coronavirus disease (COVID-19). It is an RNA type of vaccine administered in 2 doses three weeks apart through intramuscular injection. It contains mRNA, lipids, monobasic potassium phosphate, and dibasic sodium phosphate dihydrate, potassium chloride and sodium chloride, and sucrose.

Three COVID Testing Types Offered by DUVAL Pharmacy: Dunn Avenue

Duval Pharmacy is the leader in the COVID Testing space! Every test is analyzed by a licensed pharmacist who is well studied and equipped to answer your questions. We are neighbors who care about your health, your time, and stopping the spread of COVID-19. For your local convenience, Covid Testing Service is available as a walk-in, from 9 am to 5 pm Monday through Friday, and 10 am to 1 pm on Saturday.

Before You Arrive

Upon arrival for COVID testing, you will need a few things. Please remember to follow all instructions provided to make this a safe visit to your community DUVAL Pharmacy.

  • When you arrive, Go to our drive-thru Window for paperwork & payments
  • Go back to our front parking area to complete your paper works.
  • Come back to the drive-thru window to perform the test.
  • Go back to our front parking area for your wait time.
  • Call us in 20 minutes at 904-696-8882 to know your results or come back in the drive-thru to get your paper copy of the results
  • Our drive-thru has been tremendously busy so please plan for a long wait time.
  • Usually, the overall process takes about 30 minutes
  • In some special cases, we allow some of our clients to come inside the store for testing. Please have your masks on all the time while you are inside the store.

The Three Testing Types:

Fact Sheet: Rapid RT PCR Molecular COVID-19 Test

BIG NEWS!! Duval Pharmacy invested in the most accurate testing available. What used to take days is now done onsite in 45 minutes with our ONSITE PCR Device.
If you’re traveling or having surgery, you may need a PCR test. Check with your destination or surgery center to be sure. Some will accept a Rapid Antigen Test, which is less expensive, so it’s essential to find out. This is also a nasopharyngeal swab. The cost is $129. Additionally, you may submit to your insurance company to attempt reimbursement. Currently, we are unable to bill PCR Tests to Insurance Plans

  • Utilizes a nasopharyngeal swab
  • Patients can submit paperwork to the insurance company.
  • You may be reimbursed (Duval Pharmacy won’t be responsible for any denial claims)
  • Processed ONSITE
  • 45-minutes results
  • AKA Molecular Diagnostic Test

Pros: The Gold Standard Test for active diagnosis Accepted by many foreign countries, surgery centers, School Administration, etc.

Cons: More expensive than Rapid Antigen Tests

Fact Sheet: Rapid Antigen COVID-19 Test

We typically use the Rapid Antigen Test if you’re sick with symptoms or you’ve been exposed to someone who then tested positive. It’s best to wait at least three or more days from the time you were exposed before your test. This test is performed out front in your car. It will be processed onsite, and your results are complete in 15 minutes. The cost is $59. You may submit it to your insurance company to attempt reimbursement. Currently, We are unable to bill Antigen Test to Insurance Plans.

  • Utilizes a 1″ nasal swab
  • The patient may submit paperwork to an insurance company>
  • You may be reimbursed (Duval Pharmacy won’t be responsible for any denial claims)
  • Results within 15-20 minutes, onsite
  • AKA Antigen Diagnostic Test

Pros: Nearly instant results/ Perfect test for active infection & recent exposure

Cons: Not accepted by some foreign countries (check with your destination)

Fact Sheet: Rapid Antibody COVID-19 Test

If you feel you may have had COVID in the past, you would be a great candidate for this testing type. This test is a fingerstick blood draw and looks for IgM and IgG antibodies. These are the antibodies developed after one recover from COVID-19 or IgG is shown after a Covid Vaccine. It’s possible never to show symptoms of COVID and have antibodies present. The cost of the Antibody Test is $59. Additionally, you may submit to your insurance company to attempt reimbursement. Currently, We are unable to bill Antibody Tests to Insurance Plans.

    • Utilizes a finger stick blood draw
    • Patients can submit paperwork to the insurance company
  • You may be reimbursed (Duval Pharmacy won’t be responsible for any denial claims)
  • Processed onsite, Total 15-20 minutes of turnaround time
  • AKA Serology Test

Pros: Learn if you have antibodies, as you may be one of the many who had a minor infection.

Cons: Unknown whether the presence of antibodies means immunity

An Important note: COVID-19 tests are diagnostic tools. A negative test result does not rule out a person from contracting COVID-19 in the future.

How to Schedule COVID Testing Currently, We are NOT taking any advance appointments. Due to overwhelming demands, we are unable to fulfill the promised time. You can walk in to get the test done, anytime between 9:00 am through 5:00 pm on Mondays through Fridays. On Saturday you can walk in between 10:00am to 1:00 pm

From testing to recovery supplies, our pharmacists have gathered the supplies you need to recover at home. As always, if you are experiencing any of the below symptoms, you should go to the hospital or dial 911.

Symptoms that could indicate a life-threatening emergency include:

  • Shortness of breath or difficulty breathing
  • Chest or upper abdominal pain or pressure
  • Sudden dizziness, weakness, or fainting
  • A sudden change to vision or difficulty speaking
  • Confusion, a sudden change in your mental status
  • Sudden or severe pain
  • Uncontrolled bleeding
  • Severe vomiting or diarrhea

Severe COVID-19 symptoms to watch for include:

  • Shortness of breath while at rest
  • Dry cough, fever, and breathing getting more difficult
  • Significant or worrisome cough that is getting worse
  • Confusion or sudden change in mental status
  • Chest pain
  • Low oxygen levels
  • Extreme sleepiness or inability to wake
  • Blue face or lips
